Health History Form: Complete the health history form before your appointment, or allow time to fill it out on-site.

Special Note for Athletes with Special Needs: For the safety of athletes with special needs, it is recommended that your sports physical be performed by your Primary Care Provider.

EKG Availability: EKGs are not available at our location. Students requiring an EKG for their sports physical should be evaluated by their Primary Care Provider.
