Important Reminders for Your Visit

  • Cost: The cost for a sports physical is $45, payable by cash or card at the time of the visit.

  • Appointment Only: Our clinic operates by appointment only. Call or text 740-936-3050 to schedule, or book online for added convenience.

  • Parent or Guardian Required: Students under 18 must bring a parent or guardian.

  • Bring Required Forms: Please bring the school’s physical form. For your convenience, you can download the form in advance here.

  • Medication List: Have a list of all current medications on hand.

  • Health History Form: Complete the health history form before your appointment, or allow time to fill it out on-site.

  • Special Note for Athletes with Special Needs: For the safety of athletes with special needs, it is recommended that your sports physical be performed by your Primary Care Provider.

  • EKG Availability: EKGs are not available at our location. Students requiring an EKG for their sports physical should be evaluated by their Primary Care Provider.
